Band Camp Consent Form

Hi guys, Band camp will be from 2 December to 3 December.

It is compulsory for all competing members to attend.

Non-competing members are welcome to join in.

If you are coming for the camp, download the consent form, fill it up and pass it to me on 2 December.

For buglers, band will start at 830am as usual.

For the rest, it will start at 9am as usual.

For PERFORMING MEMBERS, band will be from 830/900am to 12pm. A 1hr 30mins lunch break will be given. Band will resume from 130pm to 300pm.

For NON-PERFORMING MEMBERS, band will start at 830/900am and end at 12pm.

I have allocated one person from each level to get clear plastic folders for each person in your level.
Sec 1s: Ying Shihuang Timothy Purple file X5
Sec 2s: Declan Ng Red file X 7
Sec 3s: Jeremy Yu Pink file X5

These files are for storing information such as your address, contact number, contact of next of kin.

I will need you to fill up a form and certain items will have to be filled in pen while others in pencil.

I want you to prepare the following information, files and items by next Thursday:
- Rank (in pencil)
- Full Name (in pen)
- Class (in pencil)
- A passport sized photo of yourself
- Date of Birth (in pen)
- Date you joined band (in pen)
- Address (in pencil)
- Handphone and Home number (in pencil)
- Religion (in pen)
- NRIC number [ Sec 1 and 2 in pencil, Sec 3 in pen (when we receive the pink IC)]
- Contact of Next of Kin (name and relation to you in pen, contact number in pencil)
- Medical History [in pencil (food and medicinal allergies, past injuries and operations, medical conditions)]
- The instrument that you are playing (in pencil)
- Past and present performances (in pencil)

Get all this done by NEXT THURSDAY, 11 August 2011.
